Subsequent Course Repeat

  • Prior to enrolling, this form must be approved by the School or Program that offers the course you wish to repeat.  Complete this form only for courses in the School of Natural Sciences, i.e. BIO, CHEM, ESS, MATH, or PHYS.  If you are petitioning for multiple courses, you will need to submit the form for each course.
  • Requests to repeat courses a 3rd time will be reviewed within 1 week prior to the beginning of the term for which enrollment is requested and you will receive an email when a decision has been made.
  • Do not submit form for in-progress courses; final grades must be received before submission.
  • Approvals are for a single term only and do not apply to subsequent terms.  If you are approved to repeat a course and do not enroll, a new petition for another term will be required and approval is not guaranteed.  Degree credit for a course will be given only once, but the grades assigned at each enrollment shall be permanently recorded.
  • Approval is not guaranteed.  Subsequent course attempts are only approved in exceptional cases.

** Please only submit one request per course (i.e if you need to repeat two separate courses please submit two repeat submissions forms)**

Please indicate in which term you expect to graduate.

Course numbers are between 001 to 199
Example: Spring 2019

Third and/or subsequent attempts of a course are only approved under extenuating circumstances. In a separate, typed statement please explain why you are requesting to repeat the course (beyond the course being required for your intended major), including details as to why you have been unsuccessful in the previous attempts of the course, evidence of changes in your academic behavior and goals that will lead to academic success.  Please upload your statement and supporting documentation below.  Incomplete submissions will not be considered.

Your statement should outline the factors that impeded your successful completion of the course in previous semesters and why an exception should be made. Having the course as a requirement for your intended major is not an extenuating circumstance that would warrant an exception.
NOTE: Please do not upload your MyAudit as documentation. Appropriate documentation would include those that substantiate your unique circumstances.
